Job summary

Choice Health at Home is seeking a Home Health Field Registered Nurse (RN) for PRN work in Riverview, FL. The RN will perform comprehensive home-based nursing care, coordinate with the care team, and ensure quality outcomes for patients in their homes.

Responsibilities include assessments, plan of care development, caregiver education, and ensuring safety through infection control. A valid RN license, driver’s license, CPR, and 2 years of home health experience are preferred.

Qualifications

• Current RN License.
• Valid Driver's License.
• CPR Certification.
• Valid Auto Insurance.
• At least 2 years of Home Health RN experience preferred.

Responsibilities

• Initial and ongoing comprehensive assessments of physical, functional, psychosocial, and environmental needs.
• Apply criteria for admission and re-certification to home health care.
• Implement, develop, and document the plan of care to ensure quality and continuity.
• Educate the patient and family regarding self-care, nutrition, and dietary needs.
• Determine scope and frequency of services based on acuity and needs.
• Assess the caregiver's ability to meet immediate needs on admission and during care.
• Initiate preventative and rehabilitative nursing procedures.
• Apply infection control and standard precautions in patient care activities.
• Provide directions to the Home Health Aide and LPN/LVN to ensure continuity of service.
• Prepare progress notes showing progress toward established goals.
• Inform physician and other personnel of changes in patient needs and intervention outcomes.
• Monitor cases for third party payor compliance.
• Provide effective communication to patients, family, team members, and other healthcare professionals.
