Job summary

Advanced Nursing Concepts is seeking a registered nurse to provide in‑home patient care in Ocala and surrounding areas. This PRN role involves assessing patients, building and updating care plans, administering meds, and coordinating with physicians.

Requirements include RN licensure and CPR, 1–2 years acute care, and willingness to travel. The position offers flexible scheduling, mileage reimbursement, and a supportive team environment.

Qualifications

• RN license required; CPR certification and active state license.
• 1–2 years of acute care experience; home health a plus.
• Ability to travel regularly to patient homes and locations within service area.

Responsibilities

• Assess each patient’s condition, history, and home care needs.
• Build the care plan with the patient and family.
• Update the care plan as the patient’s status changes.
• Administer medications and treatments as prescribed.
• Teach patients and families how to manage care at home.
• Coordinate with physicians and send updates at least every 60 days.
• Coordinate with community health partners involved in the patient’s care.
• Plan and carry out discharge when the patient is ready.
• Arrange any equipment or services the patient needs.
• Check in on home health aide care every two weeks.
