Job summary

Choice Health at Home in Altamonte Springs, FL is seeking a Home Health Field RN (PRN) to deliver patient-centered care in patients' homes. You will perform assessments, plan and implement care, and work with families to promote recovery and independence.

The role emphasizes education, coordination with the care team, and adherence to infection control and quality standards. A valid RN license, CPR, and a valid driver's license are required; HomeCare HomeBase experience is a plus.

Qualifications

• Current RN license and CPR certification required.
• Driver's license and auto insurance valid for home visits.
• Experience in home health nursing preferred.

Responsibilities

• Initial and ongoing comprehensive assessments and treatment for home patients.
• Apply admission and re-certification criteria for home health care.
• Develop and document plan of care to ensure quality and continuity.
• Educate patient and family on self-care, nutrition, and dietary needs.
• Determine service scope and frequency based on acuity and needs.
• Initiate preventive and rehabilitative nursing procedures.
• Apply infection control principles and standard precautions.
• Provide directions to Home Health Aide and LPN/LVN to ensure care.
• Prepare progress notes and report changes to physicians.
• Monitor cases for third-party payor compliance.
• Communicate effectively with patients, family, and health professionals.
