Job summary

Allegiance HH Northwest in Minden, LA is seeking a registered nurse to manage patient care, coordinate with the health care team, and ensure accurate documentation including OASIS and 485 forms. Qualifications

• RN licensed by LSBN with no restrictions or one year of clinical RN experience (waived for RN with LPN experience).
• CPR certification is expected.
• Proficiency in Microsoft Office and WellSky software.

Responsibilities

• Manage patient care and scheduling.
• Monitor, record and report symptoms and changes in patients' conditions.
• Maintain accurate, detailed reports and records.
• Create 485 from OASIS assessment.
• Modify patient treatment plans as indicated by physician orders and patients' responses.
• Consult and coordinate with health care team to assess, plan, implement and evaluate patient care plans.
• Instruct individuals and families on health education and disease prevention.
