Job summary

Allegiance HH West Cent LA Traditional in Many, LA is seeking a Registered Nurse to manage patient care, schedules, and care plans across home health settings. You will review OASIS data, generate 485 forms, and adjust treatment plans per physician orders and patient responses.

Collaboration with the care team and patient education are essential. Expect ongoing patient visits, documentation accuracy, and adherence to infection control protocols.

Qualifications

• Active RN with Louisiana license and at least one year of clinical experience.
• Experience with home health care and coordinating patient care teams is preferred.
• Proficiency in documenting patient records and care plans.

Responsibilities

• Manage patient care and scheduling.
• Maintain accurate, detailed reports and records.
• Process, validate, and analysis of OASIS.
• Create 485 from OASIS assessment.
• Modify patient treatment plans as indicated by physician orders and patients' responses and conditions.
• Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care plans.
• Monitor all aspects of patient care, including POC being carried out as written and goals met.
• Instruct individuals, families and other groups on topics such as health education, disease prevention, and develop health improvement programs.
• Ensure the team is educated on the needs of individuals, families or communities, including assessment of individuals' home or work environments to identify potential health or safety problems.
• Inform physician of patient's condition as required.
• Observe nurses and visit patients to ensure proper nursing care.
• Professionally manage patient, staff, and doctor issues as needed.
• Act as a liaison between the doctor and staff.
• Direct and coordinate infection control programs, advising and consulting with specified personnel about necessary precautions.
• Recommend where appropriate drugs, medical devices or other forms of treatment, such as physical therapy, inhalation therapy, or related therapeutic procedures.
• Consult with institutions or associations regarding issues and concerns relevant to the practice and profession of nursing.
• Engage in training or research activities related to nursing.
• Perform other duties as assigned by management.
