Job summary

UnitedHealth Group's Louisiana HomeCare (LHC Group) seeks an experienced Registered Nurse to provide and direct in-home nursing care under physician orders, ensuring compliance with laws and agency policies while coordinating the total plan of care with the care team.

You will perform OASIS, supervise staff, and engage families to promote safe, high-quality care. This role includes on-call and weekend rotations and requires current RN licensure in Louisiana and a valid driver's license.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to dependable transportation.
• LA-specific: 1+ years of RN clinical experience; license must be unrestricted.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with the physician and care team.
• Supervises home health aides and licensed nurses; assigns tasks per regulations.
• Provides supervisory visits and ensures compliance with policy and quality standards.
• Performs comprehensive assessments (OASIS) and medication reconciliation.
• Documents patient visits and coordinates with physicians, family, and care team.
• Participates in on-call and weekend rotation to meet patient needs.
