Job summary

Optum Home & Community Care, part of the Optum family, is expanding its primary care team to provide in-home and community-based nursing care. The RN works with NPs and PAs, supporting multiple providers on a Monday–Friday, 8:00 a.m.–5:00 p.m. EST schedule.

The role includes care coordination, documentation, and transitions in care, with travel across designated areas. A $2,500 sign-on bonus is offered for external candidates, and compensation ranges from $60,200 to $107,400 annually for

Qualifications

• RN license active and unrestricted in your state or able to obtain within 60 days.
• Associate degree in Nursing or higher (RN); BSN preferred.
• Certified in Basic Life Support or able to obtain by start date.
• 1+ years clinical RN experience.
• Driver's license and reliable transportation enabling travel up to 100% to visit clients/patient sites.

Responsibilities

• Collaborate with all members of the interdisciplinary care team on changes in condition and care coordination.
• Triage member needs to appropriate care team member and level of care.
• Assist with transitions in care: outpatient and inpatient.
• Ensure complete and accurate documentation, medication reconciliation, nursing rounds, and risk assessments.
