Job summary

Horan Hiring Solutions, LLC is seeking an experienced Home Health RN Case Manager for a field-based role serving the Dallas area. You will manage patient caseloads, perform skilled nursing visits and coordinate plans of care in collaboration with physicians and families.

The role requires recent home health case management experience, strong assessment and documentation skills, and the ability to travel locally in Fort Worth/Tarrant County. PRN schedule supporting Medicare-certified home health.

• Recent Home Health RN Case Management experience required.
• Strong understanding of Medicare home health, OASIS, plan-of-care management, and field-based nursing.
• Ability to work independently while maintaining strong communication with the clinical team.
• Strong clinical judgment, organization, documentation, and time-management skills.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care in collaboration with physicians, patients, families, and the interdisciplinary care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates appropriately.
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.
