Job summary

Ilocatum is seeking an experienced Home Health RN Case Manager for a field-based role serving the Dallas area south of Downtown and surrounding communities. This role requires strong assessment skills, solid documentation, and the ability to manage a caseload independently in the field.

You will perform skilled nursing visits, develop care plans with physicians and families, complete OASIS and visit notes, and ensure compliance with home health regulations while traveling locally in Fort

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience.
• Strong understanding of Medicare home health, OASIS and plan-of-care management.
• Ability to travel locally throughout Fort Worth / Tarrant County.
• Excellent documentation and communication skills.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care with physicians, patients, families, and the interdisciplinary team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates.
• Educate patients and caregivers on disease management, medications, safety, and goals.
• Coordinate care with therapy, social work, physicians, and leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.

Territory: Dallas area south of Downtown Dallas, including Cedar Hill, DeSoto, Red Oak, and Duncanville

We are seeking an experienced Home Health RN Case Manager for a field-based role serving Fort Worth, TX and the surrounding Tarrant County area. This position is best suited for a clinically strong Registered Nurse with recent home health case management experience, strong assessment skills, solid documentation habits, and the ability to manage an assigned patient caseload independently in the field.

Key Responsibilities

• Manage an assigned home health patient caseload in the field.

Required Qualifications

• Strong clinical judgment, organization, documentation, and time-management skills.
• Valid driver's license, reliable transportation, and ability to travel locally throughout Fort Worth / Tarrant County.
• EMR experience preferred.
