Job summary

Acloché LLC is seeking an experienced Home Health RN Case Manager for a field-based role serving Dallas–Fort Worth and surrounding Tarrant County. This position targets RNs with recent home health case management experience who can work independently in the field.

You will perform skilled visits, assessments, develop plans of care with physicians and families, complete OASIS documentation, and coordinate with the interdisciplinary team while traveling locally.

Qualifications

• Active Texas RN license required.
• Recent home health case management experience required.
• Knowledge of Medicare home health, OASIS, and plan-of-care management.
• Ability to work independently in the field with strong communication.

Responsibilities

• Manage an assigned home health caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate plans of care with physicians, patients, families, and the interdisciplinary care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates to the team.
• Provide patient and caregiver education on disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.

Territory: Dallas area south of Downtown Dallas, including Cedar Hill, DeSoto, Red Oak, and Duncanville

We are seeking an experienced Home Health RN Case Manager for a field-based role serving Fort Worth, TX and the surrounding Tarrant County area. This position is best suited for a clinically strong Registered Nurse with recent home health case management experience, strong assessment skills, solid documentation habits, and the ability to manage an assigned patient caseload independently in the field.

Key Responsibilities

Required Qualifications

• Strong clinical judgment, organization, documentation, and time-management skills.
• Valid driver's license, reliable transportation, and ability to travel locally throughout Fort Worth / Tarrant County.
• EMR experience preferred.
