Job summary

RON SUNSHINE ASSOCIATES LLC is seeking an experienced Home Health RN Case Manager for a field-based role serving the Dallas, TX area. The role requires recent home health case management experience and an active Texas RN license, with strong assessment and documentation skills for independent field work.

You will manage a caseload, perform skilled visits, and coordinate plans of care with physicians and families, while ensuring compliance with home health regulations.

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience required.
• Strong understanding of Medicare home health, OASIS, plan-of-care management, and field-based nursing.
• Ability to work independently while maintaining strong communication with the clinical team.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care with physicians, patients, families, and the interdisciplinary care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates.
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.
