CVS Health is seeking a Case Manager Registered Nurse to support dual eligible members in the Aurora, IL area. The role emphasizes coordinating benefits, care plans, and provider communications to improve health outcomes and member satisfaction.

The ideal candidate holds an active Illinois RN license, has 3+ years of clinical experience, and is comfortable traveling up to 50–75% to Kane, LaSalle and surrounding counties. A nursing degree and strong care coordination skills are essential.

Qualifications

• Associates degree in nursing required.
• Bachelor's degree preferred.
• 3+ years of clinical experience required; case management/ discharge planning experience preferred.

Responsibilities

• Facilitate benefits delivery and provide healthcare information.
• Develop, implement, and support health strategies and policies.
• Act as liaison with members, employers, providers, and insurers.
• Coordinate case management activities for catastrophic cases.
• Assess and analyze medical and vocational status of members.
• Communicate with members and stakeholders via phone or in person.
• Prepare documentation of case work activities.
• Consult with multidisciplinary teams to maximize health outcomes.
• Provide educational and prevention information for better outcomes.
• Monitor progress toward desired outcomes through assessment and evaluation.

Education

Associate’s Degree in Nursing (REQUIRED)

Bachelor’s Degree in Nursing (PREFERRED)
