Job summary

This full-time role requires 3–5 years clinical experience, an Illinois RN license, and 2–3 years in care management. Travel up to 75% across DuPage, Kane, DeKalb, Kendall areas; salary range shown.

Qualifications

• Associate's degree required; bachelor’s degree preferred.
• Minimum 3–5 years clinical practical experience.
• 2–3 years in care management, discharge planning or home health coordination preferred.
• Active, unencumbered Registered Nurse license in Illinois; CCM preferred.

Responsibilities

• Utilize critical thinking to inform case management.
• Facilitate appropriate healthcare outcomes for members.
• Assist with scheduling and accessing benefits.
• Collaborate with members and community organizations to address needs.
• Use care management tools and resources effectively.
• Work independently with virtual collaboration tools.
• Demonstrate proficiency with standard corporate software.
