Job summary

CVS Health is seeking a Care Manager in Illinois to advocate for members enrolled in Medicare/Medicaid, coordinating care with providers, community organizations and family supports to address complex health and social needs. This role focuses on collaborative, patient-centered care to improve overall wellness.

This remote/field position requires independent judgment, strong communication, and the ability to travel up to 75% from the home location.

Qualifications

• Must reside in Illinois state.
• Minimum 3–5 years clinical practical experience.
• Excellent analytical and problem-solving skills.
• Effective communications, organizational, and interpersonal skills.
• Demonstrates proficiency with standard corporate software applications.

Responsibilities

• Assess, plan, implement, and coordinate all case management activities with members to evaluate medical needs and facilitate overall wellness.
• Develop a proactive care plan to address identified issues and enhance short- and long-term outcomes.
• Use clinical tools and data to evaluate member needs and benefits.
• Collaborate with supervisor and interdisciplinary team members to address barriers and present cases at conferences.
• Interact with members telephonically or in person; may visit homes, worksites, or physician offices as needed.
