Job summary

Caresense Home Health is seeking a Registered Nurse to assess, manage, and treat patients in their homes within Pennsylvania. The role focuses on independent patient assessment, care planning, and ensuring high-quality clinical notes and progress documentation.

Responsibilities include conducting supervisory visits, evaluating patient status, educating patients and families, and preparing Plans of Care for reauthorizations. A valid PA RN license and a driver’s license are required.

Qualifications

• Home care experience preferred
• Excellent verbal and written communication skills
• Highly motivated, organizational, leadership and relationship building skills
• Ability to work in a very detailed, fast paced environment
• Currently holds an RN license in PA
• Valid state driver’s license and car insurance

• Counsel and educate patient and family to meet healthcare needs.
• Write Plan of Care for reauthorizations due for patients.
• Perform other projects and duties as assigned.

• Schedule and preform monthly supervisory visits
• Schedule and preform Quarterly Evaluations
• Complete timely and accurate clinical notes including addressing patient's progress.
• Evaluate the client status and the skills of workers while the staff member is present.
• Keeps documentation of all communication with clients, family members and support staff. .
• Provides patient, family, and/or caregiver education and information pertinent to diagnosis and safety issues.
• Counsels and educates the patient and family in meeting healthcare and related needs.
• Write 485/ Plan of Care for patients when reauthorizations are due for patients.
• Other projects/duties as assigned.

Requirements:

• Home Care experience preferred
