CareGivers America – NEPA is seeking a field-based Pediatric RN Case Manager to support pediatric patients and families across Monroe, Lehighton, and Northampton County. You will coordinate care, develop care plans with physicians, supervise LPNs, and ensure thorough clinical documentation in the home setting.

The role offers a Monday–Friday schedule, 40 hours weekly with travel time included, a company vehicle, and comprehensive benefits.

Qualifications

• Active Pennsylvania RN license (Required).
• CPR Certification.
• 1 year of professional nursing experience preferred.
• Valid driver’s license.
• Pediatric or home care experience preferred.

Responsibilities

• Complete patient assessments, including OASIS data collection.
• Develop and oversee pediatric care plans with physicians and families.
• Supervise and support LPNs and caregivers.
• Coordinate care with families and interdisciplinary teams.
• Ensure clinical documentation is accurate, timely, and compliant.
• Participate in quality improvement and staff education.
• Promote safe, high-quality care in the home.

CareGivers America – NEPA is hiring a full-time RN Case Manager to support pediatric patients and families throughout Monroe, Lehighton, & Northampton County.You’ll serve as a trusted clinical partner to families, coordinate care, and help children receive high-quality support from the comfort of theirhome.

Your Role as aPediatric RN Case Manager:

What We're Looking For:

• Active Pennsylvania RN license (Required) – BSN preferred
• Active CPR Certification

• Comfortable working independently in the field
