CareGivers America – NEPA is seeking an RN Case Manager to support pediatric patients and families with coordinated home health care. The role focuses on building strong clinical partnerships, overseeing care plans, and ensuring high-quality in-home services.

You will travel within Luzerne and nearby counties, coordinating with physicians, families, and a multidisciplinary team while adhering to clinical documentation standards and safety protocols.

Qualifications

• Active Pennsylvania or compact RN license (required).
• CPR Certification.
• 1 year of professional nursing experience preferred.
• Valid driver’s license.
• Pediatric or home care experience preferred.
• Strong leadership and communication skills.
• Comfortable working independently in the field.

Responsibilities

• Complete patient assessments, including OASIS data collection.
• Develop and oversee pediatric care plans with physicians and families.
• Supervise and support LPNs and caregivers.
• Coordinate care with families and interdisciplinary teams.
• Ensure clinical documentation is accurate, timely, and compliant.
• Participate in quality improvement and staff education.
• Promote safe, high-quality care in the home.

Are you an RN who enjoys pediatric care but is looking for a more predictable schedule and meaningful one-on-one patient relationships?
CareGivers America – NEPA is hiring a full-time RN Case Manager to support pediatric patients and families throughout Luzerne, Wyoming, and Sullivan County.You’ll serve as a trusted clinical partner to families, coordinate care, and help children receive high-quality support from the comfort of theirhome.

Your Role as aPediatric RN Case Manager:

• Complete patient assessments, including OASIS data collection
