Allegiance Healthcare of Southeast is seeking a Field Registered Nurse to join our Home Health division serving Gonzales, Livingston, Denham Springs, and surrounding areas. You will assess patient health problems, develop care plans, administer medications, and coordinate with the healthcare team to ensure optimal patient outcomes.

Qualifications

• Active RN license and CPR Certification.
• Associate’s or Bachelor's Degree in nursing.
• One to two years related experience.

Responsibilities

• Assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records.
• Administer nursing care to ill, injured, convalescent, or disabled patients.
• Advise patients on health maintenance and disease prevention or provide case management.
• Licensing or registration required.
• Direct and supervise less skilled nursing or health care personnel.
• Coordinate with health care team members and physician to assess, plan, implement and evaluate patient care plans.
• Instruct individuals and families on health education and disease prevention.
• Assess needs of individuals, families or communities including home environments.
