Job summary

Boost Home Healthcare in Warrenton, VA is seeking a dedicated Field Registered Nurse (Field RN) to provide intermittent skilled nursing care in patients' homes under physician orders and the plan of care.

The ideal candidate is a licensed RN with at least 1 year of home health experience, current BLS certification, reliable transportation, and a passion for helping patients maintain independence and quality of life at home.

Qualifications

• Licensed registered nurse in the posting state is required.
• Current BLS certification is required.
• Minimum of 1 year of nursing experience in a home health setting is required.
• Access to reliable transportation is required.

Responsibilities

• Make the initial evaluation visit and reevaluate nursing needs.
• Meet Daily Productivity Standards.
• Initiate the plan of care and necessary revisions.
• Provide services requiring substantial specialized nursing skills.
• Initiate appropriate preventative and rehabilitative nursing procedures.
• Prepare clinical and progress notes for each patient visit and summaries of care conferences.
