Job summary

Wellbound Certified Home Health Agency is seeking a Field Nurse to provide compassionate nursing care to patients in assisted living settings. You will assess health conditions, develop individualized care plans, and administer medications as prescribed.

You will educate patients and families on health management, collaborate with interdisciplinary teams, and monitor progress to adjust plans as needed. Insulin injections and wound care are part of the responsibilities.

Qualifications

• Valid RN license in New York state is required.
• Experience in home health or similar settings is preferred.
• Strong communication with patients and families is essential.
• Ability to work independently and travel between patient homes.
• Experience with pediatric patients is a plus.

Responsibilities

• Deliver high-quality nursing care in the assisted living facility.
• Assess patient health conditions and develop care plans.
• Administer medications and treatments per physician orders.
• Educate patients and families on health management and self-care.
• Collaborate with interdisciplinary teams to coordinate care.
• Monitor progress and adjust care plans as health changes.
