Job summary

Qualifications

• Bachelor's degree in social work, psychology, or related field; practicum preferred or 3+ years in social/healthcare.
• Willingness to travel 75% and work in a virtual/field setting.
• LTSS experience with seniors or disability populations is preferred.

Responsibilities

• Establish relationships with internal teams and providers; coordinate medical, behavioral, and social services.
• Manage an active caseload; track progress and ensure regulatory reporting deadlines.
• Prepare reports, update service plans, and ensure timely comprehensive assessments.
• Lead PCSP processes; assist members in obtaining LTSS and Medicaid/Medicare supports.
• Coordinate with providers and members to access LTSS, medical, housing, and other services.
