Job summary

UPMC Health Plan is seeking Service Coordinators in Fayette County for the Community HealthChoices team. This hybrid role involves daily community travel to provide comprehensive service coordination for LTSS across care settings.

The role emphasizes strong case management, in-home assessments, and collaboration with providers to improve health outcomes for seniors and individuals with disabilities. Fieldwork and HIPAA compliance are core requirements.

Qualifications

• Bachelor's degree in social work, psychology, or related field; practicum preferred.
• At least three years in social services or a healthcare setting.
• Strong case management and care coordination experience with adults.
• Willingness to travel 75% fieldwork; ability to work remotely if needed.

Responsibilities

• Establish and build relationships with internal teams and partner providers; educate on resources.
• Manage an active caseload per state ratios; track progress for OPS reporting.
• Perform administrative duties: research, analysis, reporting; update service plans and assessments.
• Lead PCSP process and assist Members in obtaining HCBS services for independent living.
• Identify and assist Members in accessing LTSS and Medical Assistance; coordinate non-Medicaid services.
• Inform Members about LTSS options, assessments, and fair hearing rights; assist with hearings.
• Collect information to inform PCSP; reevaluate care levels; assist with providers.
• Coordinate with essential entities to ensure seamless care; uphold HIPAA and regulatory compliance.
• Willingness to do fieldwork 75% and work in virtual settings.
• Support Members to maintain LTSS eligibility and explore coverage options.
