Job summary

UPMC Health Plan is seeking Service Coordinators in Beaver County for the Community HealthChoices team. The hybrid role involves daily community travel to support LTSS and HCBS services for seniors and adults with disabilities.

The role requires experience in case management, in-home assessments, and collaboration with diverse providers to improve health outcomes and maintain independence at home and in the community.

Qualifications

• Experience conducting in-home assessments and visits.

Responsibilities

• Establish relationships with internal teams and providers to coordinate resources.
• Manage an active caseload per state ratios and track progress for OPS reporting.
• Prepare reports and update service plans; ensure timely comprehensive assessments.
• Lead Person-Centered Service Planning (PCSP) and help Members access LTSS and HCBS services.
• Coordinate access to LTSS and Medical Assistance and other services; provide information and facilitate access.
• Inform Members about LTSS options, DHS Fair Hearing rights, and assist with requests as needed.
• Collect information on Member preferences and goals to inform PCSP; reevaluate care levels as needed.
• Maintain LTSS eligibility and explore funding sources, including Medicare, Medicaid, and private insurance.
• Coordinate with healthcare and support services to ensure seamless delivery; maintain HIPAA compliance.
