UPMC Health Plan seeks Service Coordinators to join the Community HealthChoices team in Blair and Cambria Counties. This hybrid role will require daily community travel to assess and support Members’ health needs.

Ideal candidates have case management and in-home assessment experience, with a dedication to helping seniors and individuals with disabilities remain independent at home while coordinating LTSS and Medicaid/Medicare services.

Qualifications

• Bachelor's degree or higher in a related field.
• Experience in social services or healthcare settings is preferred.
• Proficiency in coordinating LTSS and accessing community resources.

Responsabilités

• Establish and build relationships with internal teams and partners to coordinate medical, behavioral, and social services.
• Manage an active caseload with state-m mandated ratios and track participant progress.
• Prepare reports and update service plans to meet regulatory deadlines and compliance requirements.
• Lead Person-Centered Service Planning (PCSP) and assist members in obtaining HCBS services.
• Identify and assist in accessing LTSS, Medicaid, Medicare, and other services and supports.
• Provide information about LTSS, assessments, and fair hearing rights; protect member health and welfare.
• Collect member preferences and goals for PCSP development and reevaluation.
• Coordinate with providers to maintain LTSS eligibility and access services.
