Job summary

Qualifications

Responsibilities

• Serve as a primary contact to assist Participants and Service Coordinators via phone, email and online channels.
• Assist Enrollees in navigating LTSS services and coordinating care plans.
• Collect program data and track participant progress for regulatory reporting and compliance.
• Prepare reports and update service plans in accordance with governing bodies.
• Coordinate with care teams to obtain authorizations and connect participants with LTSS and medical services.
• Maintain confidentiality and ensure HIPAA compliance.
