Job summary

Liberty Healthcare is seeking a Protective Services Caseworker to support the Pennsylvania Adult Protective Services Program in a field-based role across Carbon, Schuylkill, Luzerne, and surrounding counties. You will conduct investigations, interview individuals, gather information, and identify risks while developing service plans and coordinating services toward case closure.

This position offers a high degree of autonomy with some flexible scheduling and potential overtime.

Responsibilities

• Traveling throughout the region with a focus on Carbon, Schuylkill, Luzerne Counties and surrounding areas
• Conducting investigations in residences
• Interviewing individuals and gathering information
• Identifying, mitigating, and eliminating areas of risk to the individual
• Documenting findings and writing reports
• Developing service plans, coordinating services, and managing cases to closure
