Job summary

21st Century Home Health Services is seeking a Field Physical Therapist to provide in-home therapy across the Davis area, focusing on building core clinical skills and supervision by a PT Case Manager. This role emphasizes follow-up visits, not case management, with leadership support for training and development.

Requirements include an active California PT license, BLS/CPR, and a CA driver’s license. Prior home health experience is preferred, with strong teamwork and documentation skills

Qualifications

• Current, unrestricted California Physical Therapist license.
• BLS/CPR certification.
• Valid CA driver’s license with clean driving record.
• Graduate of an accredited Physical Therapy program.

Responsibilities

• Provide hands-on clinical interventions in home health (gait training, balance, neuromuscular re-education).
• Evaluate patients in home setting and implement plans of care under PT Case Manager.
• Document care within 24–48 hours and maintain compliance with standards.
• Collaborate with RN, OT, ST, MSW, HHA in an interdisciplinary team.
• Meet productivity and participate in on-call/weekend rotation.
