Job summary

Columbus House, Inc. in Middletown Township, PA seeks an Outreach Case Manager to engage adults experiencing homelessness, link them to services, support sobriety, and help stabilize their lives.

This role focuses on direct street outreach, case management, and coordinating with partner agencies to provide comprehensive support. You will document cases in HMIS, attend meetings, and maintain timely charts while upholding crisis intervention, trauma-informed care, and patient confidentiality.

Qualifications

• Minimum of two years’ experience working in human services or related fields.
• Excellent organizational and time management skills with a deadline-oriented mindset.
• Strong written and oral communication skills employing the English language.
• Proficiency is Microsoft Office and other common desktop applications.
• Able to effectively use the local network of substance abuse, mental health, and social services providers.
• Familiar with criminal justice and able to engage, manage, motivate, and support.
• Familiar with mental health, substance abuse and dual diagnosis issues and group dynamics.
• Complete knowledge base of all aspects of the delivery of person-centered case management.
• Able to meet all documentation requirements.
• Knowledgeable about crisis intervention, boundaries, confidentiality, universal precautions, harm reduction, trauma informed care and motivational interviewing.
• Valid drivers’ license with a good driving history.

Responsibilities

• Conduct street outreach
• Provide linkages for clients to social, mental health, educational, vocational, recreational, spiritual, and substance abuse services
• Provide transportation to appointments for clients
• Coordinate and collaborate with other agencies and social service providers
• Engage in street outreach in all kinds of weather conditions
• Attend weekly and monthly agency, program and network meetings
• Monitor all cases and complete required documentation/charting
• Complete First Contact Forms
• Maintain weekly charting and movement reports
• Enter and maintain client charts in HMIS Case Worthy
• Other related duties to support a 17-client caseload
