Job summary

The Opportunity Alliance seeks an Outreach Case Manager for the PATH program serving Cumberland County, based in Portland/South Portland. The role focuses on outreach to homeless individuals, engagement, referral and linkage to housing and services, with a strong emphasis on recovery, trauma-informed care, and community integration.

Requires MHRT/C certification or equivalent, with two years of direct service if no degree, and offers a comprehensive benefits package for full-time staff.

Qualifications

• MHRT/C certification or a level A/B with plan to get full certification.
• At least 2 years of direct service experience if no degree.
• Bachelor’s degree in psychology, social services, counseling, or rehabilitation may be applicable.

Responsibilities

• Provide community outreach to homeless individuals in various settings.
• Identify and engage homeless persons and those at risk of homelessness.
• Link and coordinate services, housing, and mainstream resources on behalf of clients.
• Advocate for access to shelters, mental health, medical and social services.
