Qualifications

• RN license in good standing (NJ)
• BS degree in nursing or health field; or 3 years RN community/home health experience
• Experience delivering nursing services in a patient’s home environment
• Strong communication and organizational abilities
• Familiarity with computers and job-specific software programs

Responsibilities

• Assess new clients, develop plans of care, and direct level of care.
• Review client records and conduct monthly calls to ensure the Plan of Care is meeting needs.
• Perform in-person reassessments every 60 days or as needed after hospital stays or interruptions.
• Maintain regulatory documentation per NJ Board of Nursing and accrediting standards.
• Maintain good relationships with clients, families, and caregivers to support retention.
• Coordinate in-services with Director/Owner for caregiver training.
