Job summary

Allaire Health Services, LLC is seeking a Home care Field RN Supervisor (Per-Dem) to lead patient care plans in patients' homes across New Jersey. The role requires an active NJ RN license, a valid driver's license, and 1–2 years of nursing experience with home care exposure.

You will assess new patients, initiate and adjust plans of care, educate families and aides, and perform regular reassessments to ensure high-quality care in a home setting.

Qualifications

• Valid and current New Jersey RN license and registration.
• Current driver's license.
• Organized and patient oriented.
• Able to work independently.
• Good oral and written communication skills.
• Minimum of 1-2 years clinical nursing experience.

Responsibilities

• Assess patient at start of care to develop a plan of care.
• Initiate Plan-of-Care and adapt as patient needs evolve.
• Evaluate patients in their homes.
• Instruct families and home health aides in patient care.
• Reassess patients every two months and supervise home health aides.
