Job summary

A home care agency is seeking a part-time Home Care Field Nurse (RN) to join our team. You will perform in-home assessments, create care plans, and guide aides. Ideal candidates have a current NJ RN license and at least two years of nursing experience. The role offers flexible scheduling and competitive pay ranging from $35 to $45 per hour, along with mileage reimbursement and paid time off.

• Excellent clinical, assessment, organizational, and communication skills.

Responsibilities

• Maintain documentation per regulations.

We are seeking a part-time Registered Nurse (RN) to join our dedicated home care team. Our private-duty agency provides personal care (non-medical) services to the elderly in Morris and Essex counties. The field nurse will perform in-home client assessments, create individualized care plans, and orient and monitor aides providing services to our clients.

Base pay range

$35.00/hr - $45.00/hr

Job duties

• Assess new clients, develop plans of care, and orient Certified Homemaker-Home Health Aides to cases.
• Conduct client reassessments every 60 days, or more frequently if the client’s condition warrants.
• Assess Aide competency and observe their performance every 3 months.
• Maintain documentation per New Jersey Board of Nursing/NJ Division of Consumer Affairs regulations and ACHC accreditation standards.
• Be available for off-hour phone consults (very rare).
