Job summary

ForzaCare is seeking a Field Medical Case Manager to coordinate care for injured workers, connecting them with providers, employers, and insurers to ensure timely, transparent care and return-to-work plans.

Ideal candidates are licensed nurses or CRCs; prior workers' compensation experience is preferred. You will travel to appointments and work with diverse stakeholders to document goals and progress.

Qualifications

• Active RN or CRC with a college degree is required.
• CCM, CIRS or other case management credentials are preferred.
• Must comply with all state licensure and certification requirements.
• Prior workers' compensation case management experience is strongly preferred.

Responsibilities

• Coordinate care between medical providers, employers, insurers, and injured workers.
• Attend appointments with injured workers, including visits to workplaces.
• Develop and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain relationships with clients, providers, and internal teams.
