• Minimum 2 years of experience working with a social work agency.

• Conduct face-to-face program assessments for identification and evaluation.
• Coordinate waiver and BH/PH needs within the interdisciplinary care team.
• Document member goals and care plans with the circle of support.
• Support in appeals, grievances, and state audits as needed.
