Job summary

Elevance Health in Indiana seeks a LTSS Service Coordinator who will conduct assessments, develop Person Centered Support Plans, and coordinate care with clinical teams to support members with PH/BH/LTSS needs. You will travel to client sites, document goals, submit authorizations, and contribute to care rounds.

Qualifications

• BA/BS degree required.
• Minimum 2 years of experience working with a social work agency.
• Travel to worksites as necessary.

Responsibilities

• Conduct face-to-face program assessments for individuals in specialized programs.
• Coordinate and manage waiver cases (LTSS/IDD), BH or PH needs.
• Document short and long-term service goals with the member and care team.
• Identify members who may benefit from alternative levels of service or other waivers.
• Support training of new staff and participate in process improvements.
• Submit utilization/authorization requests with relevant care plan documentation.
• Report critical incidents to state and county agencies and participate in audits.
