Job summary

Elevance Health is seeking an LTSS Service Coordinator (Case Manager) to conduct service coordination for a defined caseload in Wisconsin. The role involves Person Centered Planning, assessments, and creating a comprehensive support plan to maximize independent living and community participation.

The position requires a BA/BS degree and at least 2 years of social work experience, with travel to client sites and occasional office meetings. Hybrid work is available per policy.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency; or any combination of education and experience which would provide an equivalent background.
• Preferred: waiver experience.
• Prefer comfort with using technology and travel to worksites.

Responsibilities

• Perform face to face program assessments for identification and evaluation.
• Coordinate cases and serve as single point of contact with clinical management and interdisciplinary team.
• Manage non-clinical needs of members to ensure cost-effective use of LTSS.
• Document member goals in collaboration with care teams and supports.
• May mentor or train new staff and participate in process improvements.
• Submit utilization/authorization requests with documentation.
• Report critical incidents to state and county agencies as required.
• Assist with appeals, grievances, and state audits.
