Job summary

Humana seeks a Long Term Care Functional Screen Specialist in Vernon County, WI, to support Wisconsin Family Care members. You will perform in-home assessments, schedule re-determinations, and enter data to determine level of care. Travel up to 50% and occasional office visits are required.

You must have a health-related Bachelor’s or WI RN license with experience, LTCFS certification, and strong interviewing and documentation skills.

Qualifications

• Bachelor's degree in Health or Human Services or WI RN with 1 year experience.
• LTCFS certification required; certified individuals preferred.
• Experience with Family Care target populations is a plus.

Responsibilities

• Schedule annual or change-based re-determinations to optimize time and travel.
• Use DHS screening tool to assess functional abilities, health and supports.
• Collaborate with staff to align assessment data with records.
• Conduct in-home interviews and enter data to determine level of care.
• Submit documentation per guidelines and participate in QA activities.
• Attend meetings/trainings and maintain required certifications.
