Job summary

KingRecruitingServices is seeking an experienced Home Health RN Case Manager for a field-based role serving the Dallas–Fort Worth area. You will manage a patient caseload in the field, perform skilled nursing visits and assessments, and develop ongoing plans of care with physicians, patients, families, and the inter-disciplinary team.

The role requires an active Texas RN license and recent home health case management experience, with strong documentation and communication.

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience.
• Strong knowledge of Medicare home health, OASIS, and plan-of-care management.
• Ability to travel locally in Fort Worth / Tarrant County.
• EMR experience preferred.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care with physicians, patients, families, and the care team.
• Complete accurate clinical documentation, including OASIS and visit notes.
• Monitor patient progress and communicate updates to the team.
• Provide education to patients and caregivers on disease management, medications, safety, and goals.
• Coordinate care with therapy, social work, aides, physicians, and leadership.
• Maintain compliance with home health regulations and quality standards.

Home Health RN Case Manager – Dallas, TX

Territory: Dallas area south of Downtown Dallas, including Cedar Hill, DeSoto, Red Oak, and Duncanville

We are seeking an experienced Home Health RN Case Manager for a field-based role serving Fort Worth, TX and the surrounding Tarrant County area. This position is best suited for a clinically strong Registered Nurse with recent home health case management experience, strong assessment skills, solid documentation habits, and the ability to manage an assigned patient caseload independently in the field.

Key Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care in collaboration with physicians, patients, families, and the interdisciplinary care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates appropriately.
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.

Required Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience required.
• Strong understanding of Medicare home health, OASIS, plan-of-care management, and field-based nursing.
• Ability to work independently while maintaining strong communication with the clinical team.
• Strong clinical judgment, organization, documentation, and time-management skills.
• Valid driver’s license, reliable transportation, and ability to travel locally throughout Fort Worth / Tarrant County.
• EMR experience preferred.
