Job summary

United Future is seeking an experienced Home Health RN Case Manager for a field-based role serving the Dallas/Fort Worth area. This position requires an active Texas RN license and recent home health case management experience.

You will evaluate patients in the home, collaborate with physicians and families, and ensure high-quality care in a Medicare-certified setting. You will travel locally, perform skilled visits, complete OASIS and documentation, and coordinate with the interdisciplinary

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience.
• Strong understanding of Medicare home health, OASIS, plan-of-care management, and field-based nursing.
• Ability to travel locally throughout Fort Worth / Tarrant County.
• EMR experience preferred.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care in collaboration with physicians, patients, families, and the interdisciplinary care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates appropriately.
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.
