Job summary

Henderson Group Recruiting is seeking an experienced Home Health RN Case Manager for a field-based role serving the Dallas–Fort Worth area in Texas. The RN will perform skilled visits, develop individualized plans of care, ensure regulatory compliance, and coordinate with physicians, patients, families, and the interdisciplinary care team.

Candidates must hold an active Texas RN license and have recent home health case management experience, with strong assessment and documentation skills and

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience required.
• Strong understanding of Medicare home health, OASIS, plan-of-care management, and field-based nursing.
• Ability to work independently with strong communication with the clinical team.
• Strong clinical judgment, organization, documentation, and time-management skills.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care with physicians, patients, families, and the interdisciplinary team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates to the team.
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.
