Job summary

Essenti Talent is seeking an experienced Home Health RN Case Manager for a field-based role serving Dallas and Fort Worth, TX. You will manage an assigned caseload, perform skilled visits, assess patients, and coordinate plans of care with physicians and families.

Requirements include an active Texas RN license, recent home health case management experience, strong clinical judgment, excellent documentation, and ability to travel locally.

Qualifications

• Active Texas RN license in good standing required.
• Recent Home Health RN Case Management experience required.
• Strong knowledge of Medicare Home Health, OASIS, and plan-of-care management.
• Ability to work independently with clear communication with the clinical team.
• Valid driver’s license and ability to travel locally throughout Fort Worth / Tarrant County.
• EMR experience preferred.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care with physicians, patients, families, and the interdisciplinary care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates appropriately.
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.
