Job summary

Tuesday Health, LLC is seeking a Field Engagement Specialist in Dallas, TX to enroll eligible members into its value-based palliative care program. The role spans in-person facility outreach, provider engagement, and community events, acting as a bridge among members, caregivers, facilities, and clinical teams.

The ideal candidate has experience in value-based care, provider relations, community outreach, or healthcare marketing, and will meet enrollment goals set by supervision.

Qualifications

• Bachelor’s degree in social work, public health, healthcare administration, or related field.
• 2+ years of experience in community outreach, member engagement, or healthcare navigation.

Responsibilities

• Conduct in-person outreach at nursing facilities, provider offices, and community settings to engage eligible members
• Perform telephonic outreach as needed to accomplish enrollment goals or follow up with unreachable members
• Use motivational interviewing to educate members and caregivers on Tuesday Health services
• Guide members through the enrollment process, answer questions, and address questions/concerns
• Initiate creative strategies to facilitate member contact
• Schedule initial clinical visit
• Provide ongoing telephonic customer service to members and their caregivers
• Build and maintain strong relationships with nursing home facility staff and high-volume providers
• Identify enrollment opportunities during facility/provider visits
• Collaborate with clinical team on facilities/providers needing additional education on Tuesday Health Services
• Represent Tuesday Health at local events or community health fairs as needed
• Distribute educational materials to relevant community organizations and caregiver groups
• Leverage local knowledge to identify areas of high enrollment opportunity.
