Tuesday Health is seeking a Field Engagement Specialist in Cleveland, Ohio to enroll eligible members into our value-based palliative care program. The role bridges members, caregivers, facilities, and clinical teams, with quarterly enrollment goals set by supervision.

The ideal candidate has experience in value-based care, provider relations, and community outreach, with strong communication skills and the ability to work independently in a hybrid field setting.

Formación

• Bachelor's degree or equivalent in social work, public health, healthcare administration, or related field/equivalent experience.
• 2+ years of experience in community outreach, member engagement, or healthcare navigation.
• Experience with elderly, chronically ill, or Medicaid/Medicare dual-eligible populations preferred.
• Excellent verbal communication and relationship-building skills.
• Proficient in motivational interviewing or active listening techniques.
• Highly organized and detail-oriented; able to work independently and in a team.
• Proficiency in Microsoft Office Suite and CRM/EMR systems.
• Ability to drive in day, night, or inclement weather; valid driver's license and insurance.

Responsabilidades

• Conduct in-person outreach at nursing facilities, provider offices, and community settings to engage eligible members.
• Perform telephonic outreach to achieve enrollment goals or follow up with unreachable members.
• Use motivational interviewing to educate members and caregivers on services.
• Guide members through the enrollment process, answer questions, and address concerns.
• Initiate creative strategies to facilitate member contact; schedule initial clinical visit.
• Provide ongoing telephonic customer service to members and their caregivers.
• Build and maintain relationships with nursing home staff and providers via on-site visits, newsletters, and events.
• Identify enrollment opportunities during facility/provider visits; educate on services as needed.
• Represent Tuesday Health at local events or community health fairs; distribute educational materials.
• Leverage local knowledge to identify high enrollment opportunities.
