Job summary

Tuesday Health is seeking a Field Engagement Specialist (FES) in Dallas, TX to enroll eligible members into our value-based palliative care program. You will engage with nursing facilities, provider offices, and community groups to educate and assist enrollment, acting as a bridge between members, caregivers, facilities, and clinical teams.

The role requires 2+ years in community outreach or healthcare navigation, strong communication skills, and the ability to work across multiple sites.

Qualifications

• Bachelor's degree in social work, public health, healthcare administration, or related field/equivalent experience.
• 2+ years of experience in community outreach, member engagement, or healthcare navigation.
• Experience working with elderly, chronically ill, or Medicaid/Medicare dual-eligible populations preferred.
• Excellent verbal communication and relationship-building skills.
• Proficient in motivational interviewing or active listening techniques.
• Highly organized and detail-oriented and able to work independently and as part of a team.
• Proficiency in Microsoft Office Suite and CRM/EMR systems.
• Ability to drive in daytime, nighttime, or inclement weather through assigned territory; valid driver's license with safe driving record.

Responsibilities

• Conduct in-person outreach at nursing facilities, provider offices, and community settings to engage eligible members.
• Perform telephonic outreach as needed to accomplish enrollment goals or follow up with unreachable members.
• Guide members through the enrollment process, answer questions, and address questions/concerns.
• Initiate creative strategies to facilitate member contact.
• Schedule initial clinical visit and provide ongoing telephonic customer service to members and caregivers.
• Build and maintain strong relationships with nursing home facility staff and providers via lunch and learns, newsletters, on-site visits.
• Represent Tuesday Health at local events or community health fairs as needed.
