Job summary

HealthFlex Home Health & Hospice seeks a Field Clinical Supervisor to blend hands-on PT practice with field oversight and quality initiatives. You will supervise PTs/PTAs, provide field coaching, and lead Start of Care evaluations while shaping care delivery across communities.

You will continue to see patients in the field to ground leadership in clinical reality, mentor staff, and strengthen clinical quality through trainings and performance reviews.

Qualifications

• Current, unrestricted California Physical Therapist license in good standing.
• Current CPR certification.
• 1–2 years of home health experience preferred; supervisory/mentoring experience preferred.
• Working knowledge of Medicare Conditions of Participation, OASIS, and California home health regulatory requirements.
• Proficiency with WellSky (or willingness to train) and Paylocity or similar HRIS.
• Valid California driver’s license, reliable transportation, and proof of auto insurance meeting agency minimums.
• Strong written and verbal communication skills; ability to train and mentor clinical staff effectively.
• Ability to independently manage competing field and office priorities.

Responsibilities

• Complete Start of Care (SOC) PT evaluations in the field.
• Conduct field supervisory visits for PTAs and PTs per state requirements and agency standards.
• Provide annual performance feedback and field-based observations.
• Deliver hands-on training and onboarding for newly hired therapists.
• Conduct patient care visits to support caseload coverage when needed.
• Document field encounters accurately in the electronic health record.
• Oversee 5-7 full-time equivalents.
• Participate in interdisciplinary case conferences and utilization review.
