Job summary

Ideal Option in Bend, Oregon is seeking a Case Manager/Peer Support Specialist to engage individuals suspected of low-level offenses and provide intensive case management within the LEAD Project. The role involves street outreach, collaboration with partners, and helping participants access housing, healthcare, and benefits.

The ideal candidate has experience with harm-reduction, CRM and EHR systems, and holds a Recovery Mentor or Peer Support Certification.

Qualifications

• Experience serving populations impacted by poverty and behavioral health conditions.
• Experience advocating for participants within the criminal justice system.
• Street outreach experience providing services and resources to the populations served.
• Ability to communicate effectively and problem-solve under pressure in high-stress situations.

Responsibilities

• Provide Outreach and Intensive Case Management services for assigned clients.
• Engage participants on the street and at social service provider facilities to establish a working relationship and offer services.
• Assess participants for chemical dependency and housing status; determine needs for medical, mental health, and other services.
• Assist participants in gaining access to funding programs (SSI, ABD, VA) and housing.
• Develop and maintain relationships with LEAD partners and other providers.
• Advocate for participants within the criminal justice system including court appearances and written communication.
