Job summary

CVS Health Corporation is seeking a Case Management Coordinator in Ohio. This field-based, full-time role requires travel within Northeast Ohio to member homes and locations, up to 50% of the time.

The position supports OhioRISE and focuses on care coordination for children with behavioral health needs, collaborating with MCOs, vendors, and ODM. Strong case management and community resources skills are essential.

Qualifications

• Bachelor's degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or public sector health care.

Responsibilities

• Be clinically and culturally competent in managing complex cases in the community across child-serving systems.
• Evaluate member needs and eligibility; identify high risk factors and coordinate care plans.
• Coordinate and implement care plan activities; monitor progress and outcomes.
• Provide coaching and education to empower members for independent health decisions.
• Serve as a single point of contact for members to remediate gaps in care and access.
