Job summary

CVS Health is hiring a Field-based Case Management Coordinator in Kentucky to support Aetna Better Health members. The role requires travel to member homes, schools, and clinics, and frequent in-state meetings in Louisville. Flexible hours beyond 8am-5pm EST may be needed to serve children and families.

Qualified candidates will have experience in behavioral health, social services, and a Bachelor’s/Master’s in a related field. Residency in Jefferson County is required.

Qualifications

• Minimum 6 months experience in Foster Care and juvenile justice systems, Adoption Assistance, the delivery of Behavioral Health Services, Trauma-informed Care, ACEs, Crisis Intervention services or evidence-based practices applicable to the Kentucky SKY populations, is required
• Minimum 2 year of experience required in behavioral health, social services, social work, psychology, sociology, marriage and family therapy, or counseling.
• Minimum 1 year of experience in MS Office Suite applications, including Word and Excel.
• Willing and able to travel 80-90% of their time to meet members face to face in their assigned region.
• Candidate must reside in one of the following counties: Jefferson County

Responsibilities

• Assess and advocate for member care needs using care management tools.
• Coordinate wellness plan activities and monitor progress.
• Escalate quality of care issues and collaborate with health teams to improve outcomes.
• Provide education and coaching to empower members in decision-making.
