Job summary

The Center for Urban Community Services (CUCS) in New York, NY, is seeking a Per Diem Case Manager to support housing placements and community living for clients across evening, overnight, and weekend shifts. This non-exempt role emphasizes ensuring client well-being and coordinating with teams to meet program goals.

The Case Manager will work with an assigned group of clients, deliver direct services, and help maintain program operations, including Sunday Overnight or Sunday Evening coverage.

Qualifications

• 1 year related experience is required.
• Appropriate verbal and written communication skills are required.
• Some college preferred; A. may substitute for experience.

Responsibilities

• The Case Manager helps clients obtain housing and live independently in the community.
• Works with the team to ensure program goals and contractual obligations are met.
• The Per Diem Case Manager covers evening, overnight, or weekend shifts and supports the overall recipient census.
