Job summary

CVS Health in Lake County, IL seeks a Case Management Coordinator to facilitate care coordination, education and support, with 40 hours weekly. Travel 50-75% within Lake County and surrounding areas is required, using your own vehicle where needed.

Minimum 2 years in behavioral health or human services and in case management; a Bachelor's degree or non-licensed master level clinician is required. Mileage is reimbursed under policy; benefits are provided.

Qualifications

Minimum 2 years of experience in behavioral health, social services or human services.
Minimum 2 years of case management experience.
Willing to travel up to 50-75% in Lake County IL and surrounding areas.
Reliable transportation; mileage reimbursed per policy.

Responsibilities

Evaluate referred members using care management tools and plan a course of action.
Identify high risk factors and coordinate appropriate referrals for care planning.
Coordinate and monitor care plan activities and progress.
Advise and support members to engage with providers and services.
Ensure compliance with regulatory and accreditation guidelines.
